What is a Hazard Log in health IT?

A Hazard Log is a living document maintained throughout the lifecycle of a health IT product or deployment. It records all identified clinical hazards, their estimated severity and likelihood, existing controls, residual risk ratings, and risk acceptability decisions. Both manufacturers (DCB0129) and deploying organisations (DCB0160) must maintain independent Hazard Logs. It is a core requirement for clinical safety assurance in the NHS.
