Digital-first must not mean digital-only because populations unable to access patient portals, telehealth, or online health information experience delayed diagnoses, poorer chronic disease management, and reduced preventive care. Service design should guarantee non-digital pathways alongside digital offerings to prevent inverse-care effects — where those most in need of services are least able to access them digitally.
